About the work
We are building a healthcare SaaS product for US care teams. It handles patient records, scheduling, clinical notes, care plans, and billing data. It connects to outside EHR systems and uses AI for note drafting and data cleanup.
We want one strong independent developer who owns the build end to end. No agencies. You write the code.
What you will build
Patient and provider records with role based access, scheduling with reminders, clinical notes and forms, document upload, billing data with CPT and ICD-10 fields, HL7 and FHIR data exchange with EHR systems, AI note drafting and summaries, admin dashboard with audit logs, and reporting screens.
Tech stack
Backend API: Node.js 20+, TypeScript, NestJS, Prisma, BullMQ with Redis
AI and data services: Python 3.11+, FastAPI, Pydantic, Celery, Pandas, OpenAI or Anthropic APIs, pgvector
Frontend: React 18, Next.js 14 App Router, TypeScript, TailwindCSS with shadcn/ui, TanStack Query, React Hook Form with Zod
Data: PostgreSQL 15+, Redis, AWS S3 with encryption
Infra: AWS (ECS Fargate, RDS, ElastiCache, S3, Secrets Manager), Docker, GitHub Actions, Terraform, Sentry
Healthcare: HL7 v2, FHIR R4, SMART on FHIR OAuth flow, HIPAA rules for PHI. Past work with Epic, Cerner, athenahealth, DrChrono, or Redox is a plus
Security: OAuth 2.0, JWT with refresh rotation, RBAC, field level PHI encryption, full audit trail, MFA
AI coding tools: required
You must work with AI coding tools daily. Claude Code, Cursor, Copilot, Windsurf, Cline, or Aider. We expect you to write repo rules files (CLAUDE.md or .cursorrules), break large tickets into steps the agent can handle, review every line before it reaches a pull request, and keep PHI and secrets out of these tools. If you only use AI for autocomplete, this is not a fit.
How you work
Daily written updates, small pull requests, tests as you go (Jest or Vitest, plus Pytest), current OpenAPI docs, and early flags on risk.
Engagement
Hourly, 40 hours per week, 6 months or more. Minimum 4 hours overlap with US Eastern Time. First 2 weeks are a paid trial on a small module.
