Patient Instructions Communication Page

Our current website is: https://surgicalspecialists.group This task is for Developing one page that functions to "SEND INFORMATION TO PATIENTS" The page has an UPPER and a LOWER section. Initially, this upper section should display only the header and show: ---blank--- Once patient instruction items are added below, they should appear in this upper section as sendable line items. Each saved line item should display: YouTube/video URL PDF icon if a PDF was uploaded Phone number text box Email address text box SEND button Gear/settings icon Staff can enter either a patient phone number or an email address and click SEND. If a phone number is entered, the system sends an SMS with the video link and/or PDF link. If an email address is entered, the system sends an email with the selected subject/content and the PDF attached or linked. ADD INFORMATION FOR FUTURE USE This bottom section should always remain visible and should contain one entry area with: YouTube/video URL field PDF upload box Email subject field Email content/body text box ADD button The manager can add a YouTube/video URL, a PDF, or both, along with the email subject and email body, then click ADD. After the item is added, it becomes available in the upper SEND INFORMATION TO PATIENT section for staff to use. Each saved item should have a gear/settings icon next to it. Clicking the gear icon should allow the manager to edit that item’s saved content, using the same fields as the original entry form: URL, PDF upload, email subject, and email content. 2 Sample images of how the page will look like initially with the blank upper section and later after 3 items have been added is attached... you need to host this page locally for you first and I can test it by going to a webpage you provide... when it's working fine I will give you access to our server so you can upload this document and ensure it works on our website. Our hosting is with GoDaddy. We have an account with Twilio that I can also share when you need to ensure the SMS part of the page works.
