Custom Survey & Data Collection Platform
1. Project Overview
We are seeking a developer or agency to design and implement a secure, web-based survey platform integrated into our existing WordPress website. The platform will serve as a tool for collecting initial client consultations and ongoing progress follow-ups, capturing both structured (weighted) data and qualitative (written) responses.
2. Functional Requirements
Survey Configuration: An administrative interface to create, edit, and organize survey questions.
Weighted Scoring Engine: The system must allow for the assignment of numerical values to specific answers. It must automatically aggregate these values per section and provide a final score for the submission.
Input Flexibility: Support for varied input types, including Yes/No toggles, multi-choice, and open-ended paragraph fields.
User Account Integration: Users will create accounts to facilitate the survey process. While users do not need to view their history, the system must link specific survey submissions to the correct User ID.
Workflow Automation: Ability to trigger initial consultation surveys upon onboarding and facilitate subsequent follow-up surveys as needed.
3. Security & Data Protection (Priority)
Given the sensitive nature of the health-related data collected, the developer must adhere to the following privacy-by-design standards:
Encryption at Rest: All sensitive survey responses must be encrypted in the database using AES-256 or equivalent.
Secure Transmission: Full-site implementation of HTTPS/SSL.
Access Control: Strict role-based access control (RBAC). Administrative access to sensitive data must be limited, logged, and protected by Two-Factor Authentication (2FA).
Data Minimization & Erasure: Implementation of a feature to permanently delete all records associated with a specific user upon request.
Secure Data Modeling: Use of custom database tables (e.g., wp_survey_responses) to ensure data integrity and prevent the common security risks associated with WordPress meta-bloat.
Input Security: Implementation of WordPress nonces and strict sanitization/validation to prevent SQL Injection and Cross-Site Scripting (XSS).
4. Technical Stack & Maintenance
Architecture: The developer must propose a clean architecture that does not compromise the performance of the existing WordPress installation.
Security Audit: Upon development, the contractor must conduct a security audit of the survey module and verify the security posture of the current hosting environment.
Data Retention: Configure automated deletion/anonymization for data that exceeds the established retention period.
5. Submission Requirements
Proposals should include:
Technical Approach: How you intend to structure the database and secure the data-linking between accounts and surveys.
Relevant Experience: Examples of previous projects involving secure data collection or custom WordPress development.
Security Methodology: A brief statement on your approach to handling sensitive health-related data.
Timeline: Phases for design, database modeling, development, security testing, and deployment.
Pricing Model: A high-level cost estimate or preferred billing structure.
Evaluation Criteria
Proposals will be evaluated based on the vendor’s demonstrated ability to balance ease of use for the client with the rigorous technical security measures required for health-sensitive information.
