Numerical Survey Data Collection

I need help capturing a clean set of numerical responses through surveys. The questionnaire is ready; I simply want someone to set it up, circulate it to the target audience, monitor incoming entries, and hand back the full dataset in Kobo format (the raw KoboToolbox export is perfect)...
