Fix CodeIgniter PHP 8.2 CRUD

My CodeIgniter-based site was migrated to a new GoDaddy server running PHP 8.2. Since the move the forms still render and the HTML reports load, but every CRUD action silently fails. Restoring the Add function is the top priority, because no new records can be written to MySQL...
