Update Website with Fillable Application

I need someone to update my website to include a fillable application that is a fillable PDF or Word document that they can then submit online. I also need help updating the revolution slider so it is mobile responsive, as the photo looks smashed on mobile, vs website. There is one spot on the home page as well where I need the background color to be full width in a section.
