Senior Lead Full Stack Engineer (HealthTech, Agentic AI)

Location: Remote (US product, overlap with US time zones required) Experience: 5+ years Type: Full-time / Long-term contract About Us We're a US-based HealthTech company building an AI-powered care coordination platform used by clinics and health systems. Our product combines patient-facing workflows with agentic AI systems that automate scheduling, documentation, and clinical follow-ups, reducing administrative load so providers can focus on care. The Role We're looking for a Senior Lead Full Stack Engineer to own features end to end: backend, frontend, and deployment. You'll lead a small team of engineers, set technical direction, and work directly with product and clinical stakeholders. A core part of this role is designing and shipping agentic AI features (LLM-driven workflows, tool-calling agents, RAG pipelines) into a production healthcare environment. What You'll Do 1. Architect, build, and ship full-stack features across our web platform 2. Design and implement agentic AI workflows: multi-step agents, tool/function calling, retrieval pipelines, and guardrails for clinical safety 3. Own backend services (APIs, data models, integrations with EHR systems like FHIR/HL7) 4. Build responsive, accessible frontend experiences for providers and patients 5. Own deployment and infrastructure: CI/CD, cloud environments, monitoring, and incident response 6. Lead and mentor 2-4 engineers; run code reviews, sprint planning, and technical design discussions 7. Ensure HIPAA compliance and data security across everything we ship 8. Collaborate with US-based product, design, and clinical teams What We're Looking For 1. 5+ years of professional full-stack development experience 2. Strong backend skills: Node.js or Python, REST/GraphQL APIs, PostgreSQL or similar 3. Strong frontend skills: React (or similar modern framework), TypeScript 4. Hands-on experience with LLM/agentic AI systems: OpenAI/Anthropic APIs, LangChain/LlamaIndex or custom orchestration, prompt engineering, evals 5. Solid DevOps experience: AWS/GCP/Azure, Docker, CI/CD pipelines, infrastructure as code 6. Prior experience leading engineers or owning a product area end to end 7. Excellent written and verbal communication; comfortable working async with a US team Nice to Have 1. HealthTech experience: EHR integrations (FHIR, HL7), HIPAA-compliant systems, telehealth 2. Experience with vector databases (Pinecone, pgvector, Weaviate) 3. Fintech or other compliance-heavy domain experience 4.
