WordPress Form Responsive to URL parameters

1. A new WP Form ("Step 0") that acts like a partner landing page and whose data is driven by 2 parameters to be passed in the URL. Tabular data for these 2 parameters will be pulled from a WP "database" of sorts. this form has 2 buttons - that let partners skip Section 1 (dentist information) in the case they want the device shipped to the partner. 2. Scanning the inbound URL for UTM parameters (Partner ID and Rep ID) and adjusting the new partner Step 0 of the WP form for each URL using the table stored and managed via wordpress. Partner and Rep IDs refer to partner names and rep names. All Partner data from the table auto-populates into Section 0 of the form - but can be edited in the form. 3. Verify the URL and partner IDs passed. Logic that lets the CX fail gracefully when someone uses a URL which has parameters we don't recognize. We KNOW this URL-scanning is already supported in WP Forms. 4. Hosting this mini partner data DB inside wordpress - to let the system respond fast. We want to be able to manage the partner db table in WP with a light weight admin tool in WP, or via a Google sheet that WP reads. 5. Auto-populate partner data in Step 0 ( the "responsive" WP form) . Populating data from the Partner table into the new partner section of the WP form for each (verified) URL used.
