Use AI, OCR or Python to convert PDF table of phone records into spreadsheet

This is a PDF document of phone records. I would like you to convert it to a spreadsheet. See the sample attached. The column header should be shown only once. This is a little complex because each record is three rows long. The sample is ten pages, and the full document is 3,000. My effort using Python and OCR had some success, but not perfect. Please review the sample and propose your method to do this, and a fixed price. Thank you
