Senior Full-Stack Developer (Next.js / TypeScript, Healthcare)

Looking for a senior full-stack developer for ongoing work on live healthcare apps. These have real patients, a lot of third-party integrations, and codebases that grew fast and now need someone careful to take them to the next level. The biggest thing here is ownership. This isn't a "wait for a ticket, close it, move on" role. If you're in the code and notice something that's going to break, or there's a better way to do it, speak up, even if it wasn't part of the task. Flagging a problem early beats finding out about it in production. What's needed: - Healthcare experience is a must. You've worked on telehealth or clinical apps before and understand the general flow: patient intake, provider review, prescriptions and pharmacy orders, labs, and follow-up care. - Familiar with HIPAA and how to handle patient data (PHI) safely - Experience with EHR integrations, and HL7 or FHIR data - Solid experience with Next.js (App Router) and TypeScript - Comfortable with Postgres and an ORM (Drizzle, Prisma or similar) - Real API integration work, including the messy stuff: webhooks, retries, duplicate events and things arriving out of order - Able to jump into a big codebase someone else wrote and find your way around without much hand-holding - Clean code and a habit of writing tests Availability: - About 30 hours a week - Must be available until 5pm ET on weekdays. Most of the work is async, but when something comes up, same-day communication is a must. Setup: Ongoing, with more work as projects come in. Hourly preferred, but open to discussing it. To apply, skip the long list of tools. Just share one or two healthcare projects you've worked on: what the problem was, how you solved it, and what you'd do differently now.
