Senior Software Engineer, Health App (Flutter + NestJS, HIPAA)

Higher Powers builds guided therapeutic experiences you can do at home. The product is the experience itself: composer-scored sessions with spoken word guidance, delivered through our own app, with medical screening and physician oversight handled by licensed clinical partners. We are pre-seed funded and launching in Florida in early 2027, with more states to follow. THE ROLE You will be the engineer who builds the app. Not a contributor on a team. The person. We have a finished technical spec and a fractional CTO. We do not have a line of production code. You will take the spec, pressure-test it, and ship V1 over the next six months. After launch you keep building. The second half of the job is compliance. Everything you build handles protected health information. You own the technical side of HIPAA: the PHI boundary, encryption, audit logging, access control, vendor BAAs, and the security risk assessment. Our attorneys handle the paper. You handle the systems. WHAT YOU WILL BUILD - Flutter for iOS, Android, and web from one codebase - Node.js / NestJS backend, PostgreSQL, managed cloud infrastructure on Google Cloud (GCP) - Passwordless auth with magic links and device biometrics - Embedded telehealth video with call recording into our own storage - Web-only purchasing behind a processor-agnostic payment layer - Video delivery with in-app progress tracking inside our PHI boundary - HealthKit and Health Connect biometric capture - An AI support layer with clinical guardrails and crisis escalation - Integrations with clinical systems (EHR, e-prescribing, PDMP) through APIs WHAT WE NEED FROM YOU - 6+ years building production software, with at least one product shipped end to end where you were the primary engineer - You have built or operated a HIPAA-regulated system and can explain, in detail, how you kept PHI out of logs, analytics, and third-party tools - Flutter and Dart in production, not a side project - Node.js with TypeScript, PostgreSQL, and a major cloud platform (GCP preferred) - Payment integrations and at least one video or real-time SDK - Comfortable being the only engineer: you write the code, the tests, the infrastructure, and the runbook - Clear written English. Most of our communication is async and in writing. NICE TO HAVE - Healthcare API integrations (FHIR, e-prescribing, EHR vendors) - Telehealth or payments experience - App store submission experience HOW WE WORK You report to the CTO day to day and work with the CEO. The team is eight people across the US and Puerto Rico. Remote, any time zone with 4+ hours of overlap with US Eastern. Puerto Rico based candidates are preferred, and we are open to candidates across Central and South America.
