AI Automation Expert – Fix a Bug

I have a Zapier workflow that syncs new Typeform submissions to a Google Sheet and then sends a Slack notification. The Slack step has started failing silently, notifications aren't going through, and a few rows are getting duplicated in the sheet. Started happening in the last few days, no changes made on my end. Need someone to dig in, find the cause, and fix both issues (the Slack notification failure and the duplicate rows). Deliverable: Workflow running clean, no duplicates, Slack notifications firing correctly. Turnaround: ASAP, this is blocking daily ops. Please share an example of a Zapier bug you've fixed before applying.
