Senior Full-Stack Developer (Next.js/TypeScript) for Live Telehealth Platform. HIPAA experience required. About us RxPepsDirect (rxpepsdirect.com) is a live, revenue-generating telehealth platform for physician-supervised peptide therapy. Patients pay a flat fee for a licensed medical review, complete a clinical intake, a provider reviews and prescribes, and a licensed US compounding pharmacy fills and ships directly to the patient. The platform is real and in production: live patients, live prescribing, live pharmacy integration. The codebase One production Next.js 14 (App Router) codebase: TypeScript strict mode, Drizzle ORM on Neon Postgres, Stripe subscriptions and webhooks, Vercel hosting. Integrations include an EHR (Practice Better), a pharmacy fulfillment API (LifeFile), Cal.com telehealth booking, Resend transactional email, SMS, Go High Level and n8n automations. Roughly 150k lines, 71 API routes, 20 tables. It was built fast by a technical founder and it works. Your job is to help take it from founder-built to team-grade, without a rewrite. What you will work on first 1. Patient dashboard upgrade: real-time order and shipment tracking timeline (the pharmacy data already flows in; it needs schema and UI) 2. Mobile-first rebuild of the clinical intake flow, with per-step autosave 3. Deepening the headless EHR integration (Practice Better API): per-encounter chart sync, a workspace migration with full backfill, and chart-completeness verification jobs 4. Test suite and CI (Vitest + Playwright) around safety-critical prescribing logic, plus decomposing large monolith pages into tested components 5. Labs ordering (Fullscript API) and headless live-visit scheduling through the EHR's availability and booking API Requirements -5+ years full-stack TypeScript; production Next.js App Router experience -Postgres, and Stripe subscriptions + webhooks in production -You have shipped a HIPAA-regulated product: you can talk about PHI handling, BAAs, and audit logging from experience -You have integrated at least one healthcare API (EHR, pharmacy, labs, or claims) -You write tests, and you work through pull requests Nice to have -Telehealth platform experience (Practice Better, Healthie, SimplePractice, Akute, or similar) -Playwright, Tailwind, Vercel, n8n or similar workflow automation -A designer on your team, or strong product design sense How we work -All work goes through pull requests with review (founder plus AI-assisted code review). You start in a staging environment with synthetic data; production access comes after a signed BAA and a track record. Weekly priorities call.
