We need a web-based user portal to help enroll new participants in a gun lock safety program. The portal will need the following functionality: -new user registration (requiring name/email/organization) -user authentication -password retrieval for forgotten username/passwords -different capabilities for admin and normal users -Inside the portal, users can change their First Name, Last Name, and Title. -Inside the portal, users will be able to create entities known as a "Practice" which represents the Medical Practice or organization that they belong to. -Inside the portal, users can add "Practitioners" to their "Practice". They need to add a name/email/title for each "Practitioner" they wish to add their "Practice". -Inside the portal, once "Practitioners" are added to a practice, the user is able to see an overview of all "Practitioners" in the "Practice" with the following capabilities: - The ability to email an invitation to the "Practitioner". This will generate and send an email with a unique link to the "Practitioner". The unique link will take them to a page on the website where the "Practitioner": -All of the following needs to be tracked per practitioner and viewable by the user in the overview page, such that "Practitioner" progress can be tracked. -Watches a training video (link will be provided) -Fills out a survey (link will be provided) -Provide the following resources: -Discussion Guide (link will be provided) -Clinic Toolkit (link will be provided) -The post course survey is the same as the Pre-Course survey, but only becomes available 1 month after completing the pre-course survey, and they should get an email about it. -Inside the portal, users can request materials for their practice. This should be a request form with the following questions/fields: - How many cable locks do you need (In batches of 100)? - How many rack cards do you need? - How many posters in English do you need (8.5x11 or 18x24)? - How many posters in Spanish do you need (8.5x11 or 18x24)? - How many buttons in English do you need? - How many buttons in Spanish do you need? - How many rooms do you plan to place materials in? - Any additional questions? -Inside the portal, users should be able to see past and pending material requests, and the status of their requests. -Inside the portal, admin super-users should be able to see an overview of all "Practices" and "Practitioners" as well as pending material requests. Admin super-users are able to update the status of material requests from "Under Review"/"Fulfilling Request"/"Request Filled" Clarifying questions: Multiple users can manage the same Practice. For simplicity, Practitioners will never need an account and cannot be promoted to Users. Users will need to add themselves to Practices as Practitioners if they also need to complete the training. Practitioners clicking the unique link should be able to complete all of the training/survey as a guest without having to create any kind of account. Shipping addresses should be saved permanently under the Practice profile. Users should be emailed notifications about changes to request status. Admin can add or assign additional admins. I've included a rough vibe coded mock-up of the portal and its functionality (just the portal is necessary, not the static pages). I'm not particular on the technologies used. I imagine the actual portal will be deployed on something like Vercel connected to some sort of DB. Doesn't matter too much as long as maintenance fees are low cost. The deliverables will be the code to the portal and instructions to get it live.
